Asking the bank to fix my card. Are these good ways to say?

I've entered wrong pin and it exceeded limit by the ATM and... my card is locked. I have to call the bank to clear it up. What should I say?

"Sorry, I couldn't use my credit card because I've entered invalid pins. Could you kindly unlock my card please?"

What if you want to change your pin? You head to the bank and ask:

"I'd like to change my card pin, please?

or

"I'd like to get my card pin changed, please?
