Asking teacher to complete Structured reference to assess knowledge of English

Hi

I want an example of how to write a request to my professor asking them to complete a form (a structured reference to assess knowledge of English). The request is based on the fact that I have recently finished a master's qualification that has been taught and examined entirely in English.

In the form, the professor needs to provide information that illustrates how each of the four skills of speaking, listening, writing and reading have been demonstrated or this form will not be accepted

Would you please assist me in writing my request email? I would highly appreciate it. Thanks
