Asking about someone's health

Thanks for your response to my post.

A: How are you?

B: Not feeling well?

A: What are you suffer from? / what happened to you? / What's ailing you?

Please tell me which of A's second response is natural? If none, please suggest the most natural sentence spoke in this situation.

Thanks.
