Asking about Grammar in a sentence

I saw a this title of a video on YouTube: 'Folded man' stands up straight after 28 years following surgery that broke bone.

I wanna ask that the word "following" in here, why it is "following" but not "follow". What makes it becomes V-ing

"Following" is a preposition meaning "after."

In this sentence, it is not used as the present participle of the verb "follow."
