"I asked her out yesterday."Does it mean that I invited her to go out with me (she could have rejected the proposal) or does it mean that I actually went out with her ?

It means you asked the lady out on a date, therefore you have invited her to accompany you.

'I asked her out yesterday' simply tells us that yesterday you asked her to go out with you. We don't know what her answer was.
