"He didn't work very hard as Tom did."
/I'm confused about whether Tom, unlike him, did work very hard or Tom also didn't work very hard.

I would say either:
He didn't work as hard as Tom did. (He worked, but Tom worked harder.) A comparison.
Or
He didn't work very hard, but Tom did. (He was lazy but Tom worked very hard). A contrast.
