As Per

I think I have started a thread about this phrase but I'm afraid I couldn't find it.
There are a lot of call center agents who use this in spoken and written English.
I think this is very old-fashioned.
I could remember that someone said s/he would never use/suggest this.
Any other feedback? Alternatives? Or should it be completely avoided?
How should we rephrase sentences then without 'as per'?

- "as per" meaning in accordance with, e.g. '... made as per instructions...'
