As if carrying out

1-They working slowly, as if carrying out a surgical operation.

I think there are three different (but pretty similar) possibilities here:

a-They were working as slowly as they would if they carrying out a surgical operation.

b-They were working slowly and as if carrying out a surgical operation (two independent adverbials)

c-As if carrying out a surgical operation, they were working slowly (one works slowly when one carries out a surgical operation and they were working slowly)

Is that correct?

As far as I can distinguish these, to me it means (c). However, the emphasis of the paraphrase (c) is different.
