Article - possessive

Hi,

I am unsure about articles before the possessive case in these cases.

A client's account - does the indefinite article relate to the client or account (I think account)

A/the New York Philharmonic Orchestra's fan - we're talking about a previously unidentified fan of the orchestra. I think I should go with "a", as the article pertains to the fan and not the orchestra (otherwise it would be 'the' because there's only one NY orchestra)

The children's playground - does the article refer to a certain playground that belongs to children or to a certain/definite group of children who have that playground (I think it could be both).
