Article confusions

Please help me with the following sentences:

1. "When we arrived, she went straight to the kitchen and started to prepare a meal for us."
can we say " prepare meal for us" with out using "a".

2. "Do you like the weather here? Isn't it too hot during the day but it is very cold at night?"
Why do we use "the" before the day here and not before the night?

3. "He left the home without informing anyone."
Isn't it suppose to be "he left home".

4. "The idea is simple: the attack on USA by the IJN shoot a state of war against Japan and, more important, with Germany"
Why not the attack on THE USA by the IJN? aren't we supposed to always use THE before USA , US and united states?

5. "Hurricane Andrew is one of the most devastating hurricanes ever to hit the United States. Fourteen people died because of Andrew's effects. "
Why not "THE Hurricane Andrew is one of the most devastating hurricanes ever to hit the United States. Fourteen people died because of THE Andrew's effects. "
a. Why didn't we use THE before hurricane Andrew
b. Why didn't we say because of THE Andrew's affects as we are talking about particular effects here.
