Are these sentences grammatically correct?

It has been reported that the shelf in the cabinet falls down while we were keeping the items in the cabinet.This was due to the shelf unit was mounted by improper fittings. The unit has been fixed very clumsily. It should be fixed with a proper bracket otherwise it wouldn't happen like that. We immediately informed the supplier and they accept the mistakes.
