Are these sentences grammatically correct?

Mr. Collins's surgery went well. and he will be discharged from Hospital tomorrow. He has a follow up next week.

He is planning to extent his leave till 20th of August, and also, he has some things of his own that he needs to do at his home prior to travelling. Therefore. I request you to kindly grant him this leave extension.
