Are these sentences correct?

As requested by the consultant, we mock-up a chandelier for the living room in our warehouse premises. After their review, we dismantled the chandelier but unfortunately the crystal arm was damaged by one of our technicians while he was trying to dismantling the chandelier, so please ask the supplier to replace the damaged part immediately.
