Are these sentences correct?

I regret to inform you that due to my poor health that I am resigning from my job of xxxx and will no longer be able to continue to perform my duties at this company

I am uncertain if you are aware that I have been ill for quite awhile. I have been advised by my doctor that I am no longer well enough to continue working, so after careful consideration I reached the decision that my resignation is the best conclusion for myself, so my last working day will be on [date]

It is, therefore, requested that I may kindly be relieved from my post on the requested date.
