Are these sentences correct?

Passive voice

*Present perfect continuous

Book has been being written by me.
Book has not been being written by me.
Has book been being written by me?

*Past perfect continuous

Book had been being written by me.
Book had not been being written by me.
Had book been being witten by me?

*Future continuous tense

Book will be being written by me.
Book will not be being written by me.
Will book be being written by me?

*Future perfect continuous tense

Book will have been being written by me.
Book will not have been being written by me.

I would add that except as a grammar exercise, it's hard to imagine these sentences actually being used. People would be much more likely to say, "I have been/had been/will be writing the book."

I have also seen "a" being used in the beginning of passive voice sentences. Is there a rule which we have to follow regarding the usage of "the" or "a"?

No. Passive voice has nothing to do with it.

If you have a concrete countable singular noun like book, you have to use either the or a(n). Those sentences could have used a book instead of the book. This applies in both active and pass
