Are these sentences correct ?

On [date], Mr. X rushed to his home town as his father admitted to the hospital in very critical condition. On [date], his father died due to the critical illness.
As per the company policy, we have enclosed the copy of the death certificate, copy of the travel tickets and copy of the insurance policy for your kind perusal
Therefore, in accordance with terms and conditions of his insurance policy, we would be grateful if you could arrange for the cash grand for travel fare at the earliest.
