Are these sentences all have the same meaning?

Are the following 5 sentences equal to one another or if they have different meanings?
If so, can any one tell me what the diferences are?

a. He lay in bed and read a novel.
b. He lay in bed to read a novel.
c. He lay in bed reading a novel. < no comma >
d. He lay in bed, reading a novel. < with a comma>
