Are these senteces correct?

Please note that one of our staff members, Sanchez has made request for emergency leave as he is suffering from Hernia. His doctor advised him to undergo the surgery immediately, so I am planning to relieve him end of this weekend. Also, his annual vacation is due to be taken in March 2014
