Are these reasons grammatically correct?

Write reasons why Cindy may call.
bank--- Cindy needs money.
dentist---Cindy has a toothache.
shoe store----Cindy needs new shoes.
doctors---Cindy is sick.
optometrist----Cindy is near sighted. Cindy has eye problems.
hospital--Cindy has a stomachache.
DMV---Cindy needs a driver's license.
fire department----Cindy's house is on fire.

near-sighted or nearsighted The rest are fine, grammatically. However, I doubt that anyone has phoned a bank to ask for money, phoned a shoe store because s/he needs shoes, phoned a hospital to complain of a stomachachel or phoned the DMV for a driver's license. IN real life we just go there.
