Are the sentences correct?

1. "The doctor said that I was fine to go back to school, as long as I agreed to outpatient counseling."

2. "The doctor said that I was fine to go back to school, as long as I agree to outpatient counseling." <- I think this sentence is wrong?

Whether and which words to backshift in reported speech is not always clear cut.
You are just about always correct if you do (I was fine, I agreed).

In your second sentence, let's say he said it today and you have not yet gone back to school, you could say "...that I am fine to go back to school as long as I agree..." but don't mix "was fine" with "I agree."
