Are the commas needed?

Your doctor may, however, prescribe a different dose.

Are the commas obligatory?

Thanks.

Yes, they are obligatory. However, you can get rid of one of them by placing however at the beginning of the sentence, as I have done here.

I forgot to ask another question.

Can 'dosage' be used in place of 'dose' in the sentence?

Thanks.

I believe that dose is used for general reference, as: I forgot to take my dose of medicine today. Dosage refers to the prescribed amount. So, yes,, I think that dosage would be better in your sentence.
