Are all the answers possible?

Hi teachers,
How often was her cat in the kitchen at lunch time?
a) Always.
b) It was always.
c) It was always in the kitchen.
d) It was always in the kitchen at lunch time.

A is the most likely. B would not be used. C probably means that it was there during lunch and at other times too.
