approval on leave

I want to take 2 days off.According to my company rules if I don't take it, it will be expired this month.please correct me if I'm wrong.

Dear Ms. Manager,
I would like to take annual leave on September 18 and 25, 2006. Please kindly approve it.

I would like to seek permission for 2 days leave from 25th to 27th March 2016 i.e. on Good Friday to Easter Sunday.
