Apposition without an article

Evidentiary material 4, the original goods customs declaration form, a photocopy of the shipping order and the consignment booking note counterfoil

May I know for this apposition, why the first definite article immediately after the comma is not necessary ( the original goods customs declaration form) but thereafter the indefinite article (a photocopy) and definite article (the consignment booking note counterfoil) is allowed?
