I would like to inform you that in the on going SSC examination the students of our school, XYZ, are having the question paper where their roll numbers are misprinted. They are writing their roll numbers on their section "A" question papers by hand. This may affect the result of our students adversely.
I, therefore, request you to take necessary actions as soon as possible.

I am guessing the R/Sir means "Respected Sir," but it is not used in my part of the world.
