Application form

In an application form there are two fields(among others) as follows:
Name of the family member
Relationship with the member

Now given that the member is my father should I put 'father' in the second field or should I put 'son'?

If the second sentence follows the first, I believe they meant to say: Your relationship with this member (which would be "son").
