Application for my medical leave

Question

hi,i had an ovarian cyst operation last month and apply for my medical leave, and i promised to resume at work after a month, 24 Sept.2012 when i did my opertion and supposed to resume on 24th of Oct, but unfortunely my doctor advice me not to go to work until my wound is perfectly healed, as of now my conditon is still not permiting me to do my work which i need 9 hrs standing..im worried to lose my job. i want to request another 15 days but i dont know how to write letter to them...i dont wnt them to get dissapointed wth me as  i am new wth this company..please anyone help me...i really appreciate if i can anyone can help me for this..

As the wound has not been healed completely, I have been advised by the doctor to take complete rest for another 15 days. I, therefore, request you to grant me medical leave for another 15 days with effect from 25^th October 2012 to 10^th November 2012.
I am enclosing a medical certificate from my doctor for your ready reference.

As the letter is addressed to the Manager, you need not to write it. Copy the letter as it is and submit the same to your manager either in person or by any of your colleague.
