Application for midwifery training in UK universities

A friend of mine is currently pursuing a Bachelor in Midwifery and, in order to obtain her degree, she needs to do her training as a Registered Midwife during the last semester of the degree course. So she has chosen to apply to universities in the UK. Unfortunately, she has no idea what her e-mail application should look like (structure, information included, etc.) and has not been given any guidelines from her university either. I am sure that there should be a certain form for such applications and that the candidates should stick to it.

Would it be possible for you to refer me to a sample letter? I have already searched the Internet, but, so far, I haven't come across something that suits this case. I would be really grateful for any information or tips you may have to offer.
