APPLICATION for Dental leave

hello everyone i want someone here to write an application for
subject is that : i was not going to school because of my tooth ache so ultimately i have to remove my lower moler so i was not able to goto school for the next 2 days bcoz of bleeding therefore anyone could write an application regarding this subject

i 'll be very thankful .i m not write it bcoz my english is not very good so anyone plz write an application for me

i hAve to give this wiht my medical certificate attached therefore plz write about it too

This letter is to formally request leave of absence for medical reason. because i will be having a tooth pain/tooth ache during worked hours and i am not doing well with my health condition.upon my consultation the dentist said told me i have to remove my first molar which is lower jaw and second molar which is upper jaw so that i have been advised completed bed rest for 15 days.
