Application for alteration of working hours. Pls help me check, tq.. ^^

Alteration of working hours.
With respect to the above subject, I would like to appeal for the alteration of working hours. Following my current study plan, adjusted working hours will be as follow with effect from 19th August till 30th November.

Tuesday : 8.30am - 1.00pm
1.30pm - 5.00pm
Saturday : No work.

I hope you could approve my proposal. Thank you for your time and assistance in this matter.
