Appears

She is not as old as she looks.

If it was 'appears' instead of 'looks' as this:

She is not as old as she appears.

would it still work? If it worked, would it mean exactly the same?

Sounds like the focus of 'appear' is more on physical appearance. Good.

Then what about this?

She is not as old as she seems.

If the sentence above worked, would it have exactly the same meaning as the one with 'appears'? If not, what would be the difference?

The meaning with looks, appears, and seems is similar, but there are subtle nunaces so that they would be used in different situations:

A: How did the date go with the girl I set you up with?
B: I think she's too old for me.
A: She's not as old as she looks. She's only 33, a yr. older than you.
