apology letter

I work for a company that produced a book for a school. They created, deisgned and published this book. After printing 300 copies and sending them out, the school noticed several grammer and spelling errors. They would like us to write a letter of apology to isert into each book. I NEED HELP!!! This is what I have come up with.

Dear Students, Parents, Teachers and Staff.

Please accept our apology for any grammatical errors in this year’s yearbook.

Please note that these books were solely created and assembled by ___________.

We sincerely hope you will enjoy the memories they may bring.

Sincerely ,

?????????? (do I put a name or company name?)

I did not know If i should make mention that the school did not "have anything do with this" or how to put that in their. Any help would be great.
