I need to write an apology email to a fellow co-worker. There was a miscommunication during shift- hand off yesterday and I emailed her about it and she took offense to my email. Can you help me?

It sounds like I offended you and for that I apologize. I didn’t mean to offend you. I was just lost when it came to this appt. You usually do pass on information to me about matters such as this and I was confused as to why there was nothing on this appt in the handoff. I did not mean to sound as though I was pointing the blame at you about this appt. Again, I apologize if my first email made you upset.
Thank you,
