Dear Anthem,
Summer 2009, I went to Africa for summer vacation. While on vacation I injured myself playing basketball. I severely subluxated my ankle and went to the nearest hospital I could find. Fortunately, the hospital was for expatriates and luckily for me they were also partners with Anthem Blue Cross.
I have enclosed all the documents that were given to me for/from my visit and hopefully I can have the expenses reimbursed to me.
Feel free to contact me if there are questions or should you need to speak to me about what happened at the time of the injury.
Please help me proofread.
