What do you mean by 'ex'? ' is asking about someone's occupation. If the answer 'He plays soccer' is therefore an unlikely one.

We assume that, if there is no further context, that the question 'What does he do?' is asking about someone's occupation. If the answer 'He plays soccer' is therefore an unlikely one. If he does that for a living, then the answer will be 'He's a footballer' or 'He plays for (for example) Manchester United'.

That would only work if you were clearly talking in a sports context.
Or if you said eg What does he do every Sunday afternoon?

More natural is eg What sports does he do?
