To whom it may concern,

I am writing this letter to apply for the annual leave of four weeks which I can avail as per the company policy. I have not availed any of the leaves from past six months so I would like to apply for leave starting from ( March ,2018).

The main reason of this leave is because I want to celebrate my birthday with my family especially with my mother, and besides my younger sibling will also come home to the Philippines, I didn’t see her for a long time and I think that would be the best time to see and bond with her. Our mother will be very happy to see us both.

I will make sure that I hand over all my responsibilities to another employee and explain everything to him/her.

I will be able to resume with work from ( date) and I hope that there is no emergency or need for extension else I will inform about the same. This is my contact details please contact me for any information

Are you sure it is necessary to add so many details? In opinion it is better just to name the reason an explain a bit more, without telling the whole dramatic story

I am sure that "to whom it may concern" has no interest whatsoever in your birthday or family. Including those details is very unprofessional.

To whom it may concern,
