annual leave

This letter is regarding my annual leave for 37 days which i already requested and slots were availble for 7 days in the month of oct from 9th -17th and the rest of leaves 30 days in the month of nov.since i was pregnant my expected date was on nov 1st asper our company rule there is 45days of maternity leave which i started on sep 17th and will get over by nov 1st now so I miss that 7days which was already scheduledin oct where  I am requesting that on december.here goes the letter,pls check and correct me with the gramar.
Dear sir,
I would like to request for my annual leave(37days)commencing from 2nd nov-8th dec,as discussed earlier with wfm(work force management) they had slotted 7days in the month of october and the pending 30days in the month of november from 1st nov to 30th for me,since I delivered before my expected date I started with my maternity leave(45days) on sep 17th which ends by nov 1st  where  my 7days of annual leave I am requesting in december.pls check and confirm waiting for responce at earliest.
