Annual Leave

Hi i am to apply for the first time in 3 years for my annual leave. below is what i hav put together, please rectify my mistakes and if there is any thing to add do not hessitate to help.

I am writing with regard to the annual leave of four weeks that i can avail as part of my employment with this organization. I have not yet availed my annual leave and wish to do so for a period of four weeks starting 26th April 2011.
