"and" or "or"

Dear friends,


I was reading a travel insurance policy which states: "You will be entitled to consultation with a doctor for a maximum of 10 visits per Hospital Confinement or day surgery procedure"


I would like to know if the 10 visits apply to both the Hospital Confinement and day surgery procedure (ie 10 visits only in total), or I'll be entitled to 10 visits to each Hospital Confinement and day surgery procedure (ie 20 visits)?

It's a bit confusing...

You will be entitled to consultation with a doctor for a maximum of 10 visits per [Hospital Confinement or day surgery procedure].

I.e., whether you are kept in hospital or have day surgery, you are entitled to 10 visits in either case.
