Analyses of a sentence

Believe me, I know first hand that a large number of artists go unseen or unheard their entire lives.

I'd like to know here what role "first hand" plays in the light of the context.
And I'd like to know if "for" is implied before "their entire lives."
Thank you in advance for your help.
