an improvement vs improvement

Those with a particular version of the COMT gene saw AN improvement in their health after placebo acupuncture.

What would be the difference in meaning without "an" before improvement?

In addition, doesn't "acupuncture" require an article or have to be in plural? For example, we say "I had an MRI" or "After the MRI, we can tell..."
