Sunny, you asked why I preferred (D). You have a mixture of words, phrases and sentences, so (A), (B) and (C) are not completely correct (except in a loose sense of "words"). I also don't see any extra value in using a generic word like "items" over just saying "the following".

FYI, I am a native English speaker from the UK.

This is not very natural. If you want to say it this way, it would be better to use "for" rather than "of".

I guess, though, there is a distinction between the overall exam instructions, explained at the start of the exam, and the instructions for individual questions.
