An email to supply information to Uni

I am trying to send an email to supply some information to an university.

I wonder if I am writing it politely and correctly.

Here is my email:

Dear Sir or Madame,

I am writing to supply the details of the percentages achieved in the XXXXX A-level modules that I have taken.

My Ucas personal ID is XXXXXXXXXX.

I achieved XX% in module 1, XX% in module 2, XX% in module 3, XX% in module 4, XX% in module 5 and XX% in module 6.

Should I write something here?
