American?

Does American include someone from South America and North America? Or does it only include those who are from the US?

If it only includes someone coming from the US, is there any collective term denoting someone who is from a South America or North America country, but not necessarily from the US?

The British use it both ways. Americans don't even believe that when you tell them. To us "Americans", it only means a citizen of the USA.

This is awkward. It would be more natural to write someone who comes from the US. You could just write someone from the US. You can choose one of those suggestions.
