Amend my letter and make it as professional way

Hi,

Anyone can help me to amend my letter to make it as professional letter. Actually I am writing a letter to the Immigration officer for not producing certain documents. The letter is as follows...

Dear Immigration Officer,

I would like to apply long-term visit pass for my wife and son. In order to appy for long-term visit pass, CPF contribution history for the last 12 months is required which is not possible for me to produce since I obtained my Singapore Permanent Resident in October 2004. Consequently, I can produce last 3 months CPF contribution only.

I seek for your kind co-operation to approve the above-mentioned application
