Am I phrasing this right?

hi, I want to know if this is the right way to phrase this sentence

"I told you what it meant already. Why are you asking me again?"

Is meant used here or should it be mean?

Also..

I would have yelled at him if he hadn't passed the exam.

Should I use didn't or hadn't?
