Already with past perfect - future perfect

Hello.
Is it awkward or ungrammatical not to put already with the past perfect and the future perfect?

The patient had (already) died whe the doctor arrived.
When you get back, I will have (already) painted the bedroom.

And which is more idiomatic? With or without already? Do you have any preference?

Hi,

And which is more idiomatic? With or without already? Do you have any preference? Already.

'By the time' is also idiomatic.

The patient had (already) died by the time the doctor arrived.
By the time you get back, I will have (already) painted the bedroom.

Why is that idomatic?

By the time is an idiom?

Hi,

I just meant that using 'already' in this way is characteristic of the way people speak.
