In the context I think it means that, in case it had been Sid, who had been in the good books of aunt Polly, aunt Polly would have felt happily relieved thinking Sid would be take care of himself and since it was Tom who was feeling sick, she was worried to leave it - taking pain killers- him as she did not trust him fully.
