All right

Good evening.
Is it true that when you answer the question: 'How are you?' by saying 'I'm all right' you are not so enthusiastic about your condition?
Does it sound like 'I'm ok but I could be better'?

It very much depends on tone of voice. If you say it in a trailing-off voice then it sounds unenthusiastic. If you say it with a rising and emphasised "all right" then it is more enthusiastic (but probably not quite as much so as "Very good" or "I'm great" or something like that).
