I am always confused about the use of 'all'.
For example, which of the following passages is correct?

The article will not be published unless it is approved by...
a) ..all reviewers.
b) ..all the reviewers.
c) ..all of the reviewers.

Is there any difference in meaning? Are there any rules regarding the use of "all" in such contexts?
